Family Physicians

As the primary touchpoint in your care journey, family physicians guide and coordinate access to the services you need. From specialist referrals to follow-up care. They also take a proactive role in disease prevention and health promotion through routine screenings, vaccinations, and personalized lifestyle support.

Nurses

Nurses play a vital role in delivering comprehensive care. Registered Nurses (RNs) and Registered Practical Nurses (RPNs) both provide health assessments, chronic disease support, and patient education, while coordinating care within the team and leading health promotion efforts in the clinic and community. They also support patients with stable conditions by administering medications, providing wound care, and reinforcing care plans.

Registered Dietitian

Registered Dietitians (RDs) use their expertise in food and nutrition to support patients’ overall health. They assess dietary habits and create personalized nutrition plans to help manage conditions like diabetes, heart disease, digestive issues, and food allergies. RDs also lead community education and nutrition programs, promoting healthy eating habits and addressing local health needs.

Social Workers

Social Workers support patients’ mental, emotional, and social well-being as part of the primary care team. They provide counseling, connect individuals with community resources, and help navigate challenges such as stress, grief, trauma, and life transitions. Social Workers also assist with system navigation, housing, financial concerns, and crisis intervention to ensure holistic, person-centered care.

Psychologists

Psychologists support mental health through evidence-based assessment, diagnosis, and therapy. They help individuals manage a range of concerns, including anxiety, depression, trauma, and behavioural challenges. Working as part of the primary care team, psychologists provide individualized treatment plans and collaborate closely with other providers to promote emotional well-being and overall health.
